A. Create a Google form
First of all, you need an activated Google account in order to create a Google form.
On your browser, go to the URL http://gmail.com/
On the top right corner, click on the symbol with 6 small circles (as shown in picture), choose
Drive.
Click on New button, then choose Folder to create a new folder used to stores all the test. This is
an optional step as its aim is to keep your workspace as clean as possible.

Give the folder a name.

Inside the have-just-created folder, select New button, Click on More, Google Forms, and go to
Blank form.
You will be redirected automatically to the untitled form.
B. Setting up the form
Rename the form, the display title and the description.

On the top right corner, click on the Settings symbol next to the Send button.
In General tab, adjust the settings as you wish. However, make sure that:
- The checkbox of Limit to 1 response is ON.
- The checkbox of Edit after submit is OFF.

In the Presentation tab, shuffle question order is recommended to be enabled.
In the Quizzes tab, make sure the switch of Make this a quiz is enabled. Save all settings.

C. Create questions
1. Create multiple choice question.
Click on the area of available question to start editing.
First, select the question type - Multiple choice
Second, edit the question text.
Then, input the answers.
Next, enable the Required switch.
After that, click on Answer key to set to correct answer and the points for this question.

In order to create another question, click on the button with the plus symbol.
2. Create short answer question.
The manipulations are similar to the ones in create multiple choice question, but the question
type should be Short answer.

Note that the key answer is NOT case-sensitive. The capital letters and the small ones
would affect the result.
3. Create checkboxes question
With checkbox question, there are more than one correct answers.
The manipulations are similar to the ones in create multiple choice question, but the question
type should be Checkboxes.

D. Preview the test.
After complete creating the test, click on the button with the eye symbol to preview.

This is the test view from a student.

After submission, student can view their score (if the release grade in Quizzes setting is
Immediately after each submission). Otherwise, they cannot see until the author release the
score.
The View score option would display like this.

E. Send the test.
Click on the top right Send button.
There are two ways.
The first option is emailing all students.
On the Send via, select the envelop symbol. Type or paste all the students’ emails in the
To textbox, each email is separated by a comma. Click on Send.

The second one is sending via a sharable link.
Click on the link symbol next to the envelop one. Click on Shorten URL, then choose
Copy.
Paste the link in the platform where you want to announce your students about the test
(e.g., Blackboard, MS Teams, etc.).
Note:
- Students are required to login in order to take the test.
- Student who tries to have another attempt would receive the message as follow.

F. Collect the submissions
In the form, click on Responses tab. The number next to the Response text is the number of
submissions.
Check individual attempt
To review student’s response one by one, click on Individual tab. Select the student to review
his/her attempt. If nothing arises, click on Release score to announce the studentsscore through
their email.

You can give each student your feedback in Message textbox, click on SEND EMAIL AND
RELEASE.

Score would be announced to students’ personal email.
Collect data from Google Sheet
In the Responses tab, click on the Google Sheet symbol.

The Google Sheet will be automatically opened. It describes the students responses in detail
including the timestamps, email addresses, scores, and students answers.
The Google Sheet file locates in the same folder that contains the Google form.
